Independent Therapeutic Leave Package

Package includes a sample policy and screening tool for residents desiring to leave the facility and go outside in the community without supervision/assistance from the facility team. The screening tool addresses the resident’s safety from a variety of perspectives including cognition, behavior, functional, and is designed to be completed by the IDT with approval of the team’s recommendation by the attending practitioner.
